This page will guide you through what to prepare, address your concerns, and explain my secure submission process.
Preparing for a seamless submission
Having a few key items ready before you begin your non-test (fill-in style form) submission will be helpful:
- Learning overview: Be ready to share a general idea of your child’s progress, strengths, and any areas of growth.
- Subject coverage: Share a brief overview of main resources used and concepts covered from core subjects such as reading, writing, math, science, and social studies.
- Samples of work: Select work that reflects your child’s learning throughout the year. This can include a mix of assignments, writing samples, math work, projects, photos of activities, or even online learning results.
- Digital files or photos: Have your files organized and ready to upload. Clear photos or PDFs work great.
Addressing common concerns
I understand you might have questions or concerns. Here are some of the most common ones I hear from parents:
- Privacy and security: All submissions are kept confidential and are only reviewed for evaluation purposes. Your child’s information is never shared.
- "Am I doing this right?": There is no “perfect” form—just an honest reflection of your child’s learning.
- What if I do not have enough?: You do not need a large volume of work. A few meaningful samples that show progress and effort are more than enough.

Digital Google Form:
Utilize my secure Google Form for direct uploads.
- *Note: Using a Gmail account, you can type in the provided digital form, responses will autosave for up to 30 days (if signed in), upload up to 10 attachments of your student's work through the year, and submit.
Digital Microsoft Form:
Submit via my secure Microsoft Form for another easy-to-use option.

$25/individual or $20/individual for 2 or more in the family.
What happens after you submit your child's non-test (fill-in style) form?
Once you submit your child's digital non-test (fill-in style) form or email the PDF option, you will receive a confirmation email. Following my review, I will send an official evaluation letter for your records, completing the assessment process.
